YEARLY PHYSICALS AND UPDATED IMMUNIZATIONS ARE NEEDED TO KEEP ALL RECORDS UPDATED AND IN ORDER.

PLEASE SEND IN APPROPRIATE DOCUMENTS AS SOON AS POSSIBLE.

FLU SEASON IS HERE!!!              9/21/2020

PLEASE , If your child has any flu symptoms,keep them home from school.  All students are required to receive the flu vaccine by December 31, 2020 as mandated by the State of Massachusetts for the 2020-2021 influenza season, unless either a medical or religious exemption is provided. Also exempted are K-12 students who are homeschooled and higher education students who are completely off-campus and engaged in remote learning only. This new flu immunization requirement to enter school in January is in addition to existing vaccine requirements for all those attending child care, preschool, K-12, and colleges and universities in Massachusetts. Elementary and secondary students in districts and schools that are using a remote education model are not exemp

Students should be fever free for 24 hours without medicine, before returning to school.

TO ALL PARENTS'

PLEASE CHECK YOUR CHILD'S BACK PACK FOR A NOTICE FROM THE NURSE. SOME OF OUR STUDENTS ARE IN NEED OF UPDATING THIER MEDICAL RECORDS. IF YOUR CHILD IS ONE OF THEM, THEY WILL RECEIVE A NOTICE FROM ME.

PLEASE BE AS PROMPT AS POSSIBLE AT SENDING BACK THE INFORMATION NEEDED . 

AS ALWAYS, ANY TIME YOUR CHILD GOES TO THE PEDIATRICIAN, PLEASE GET COPIES OF ANY UPDATED INFORMATION AND SEND TO ME.

MEDICATION PICK UP        6/5/2019
 
ALL STUDENTS MEDICATIONS MUST BE PICKED UP IN THE NURSE'S OFFICE BY JUNE 13TH.
IF NOT PICKED UP BY THAT DATE, MEDICATIONS WILL BE  PROPERLY DISPOSED OF.

IF YOUR CHILD REQUIRES MEDICATION DURING THE DAY FOR THE SCHOOL YEAR 2019-2020, ALL MEDICATION FORMS MUST BE FILLED OUT AGAIN WITH PARENT SIGNATURE AND LICENSED PRESCRIBER'S ORDER AND SIGNATURE.
FORMS CAN BE OBTAINED FROM THE NURSE OR ON THE SCHOOL WEB SITE UNDER PARENTS PAGE/NURSES OFFICE.

I AM SENDING A NOTICE HOME TODAY FOR THOSE STUDENTS ENTERING KINDERGARTEN, 4TH GRADE AND 7TH GRADE IN SEPTEMBER.
THIS NOTICE IS FOR THOSE STUDENTS NEEDING TO HAVE THIER HEALTH RECORDS UPDATED, PER STATE LAW, BEFORE ENTERING THOSE SPECIFIC GRADES.
THIS NEEDS TOO BE DONE BEFORE THE START OF THE NEW SCHOOL YEAR.
 
ALL OTHER STUDENTS ATTENDING MATERDOLOROSA SCHOOL ARE REQUIRED TO PLEASE SEND IN UPDATED PHYSICAL AND IMMUNIZATION RECORDS AS YOU RECIEVE THEM.

 HEALTH RECORDS  10/30/2018
 
PLEASE  check your child's back packs for reminders from the Nurse. Many health records are missing updated physicals and immunizations, etc. If your child receives a reminder , please be prompt in getting those documents to me . If you have any questions , call me any time.
It is a state law to have updated records in all schools.

WELCOME TO THE NEW 2018-19 SCHOOL YEAR!!!
 
Just a few reminders for Parents/Guardians,
1. For those students who need medication ie: EPI pens, inhalers, etc.....Please obtain medication forms on line (above) or in my office, before bringing meds to school. M.D. and parent/guardian need to fill out thier portion.
2. When you bring your child to the Physician for physical or Immunizations, please obtain a copy of those documents for the health record at school. I need to keep each one updated.
3. If your child is exempt from any immunization for medical or religious reasons, I need to have that in writing each year.
4. Please check this page often as I update it as needed.

1/10/2018                        HAPPY NEW YEAR!!!!!
 
To all parents and guardians:  PLEASE , if your child is sent home for vomiting or diarrhea, they should not return the next day. They need to be free of symptoms for 24 hours before returning to school.  
We are trying to keep all of our students and staff as healthy as possible during the flu season.
 
If you have any questions, do not hesitate to call the school or myself or refer to "When should I keep my child home" form, which can be seen and printed at beginning of page.

10/26/2017  MEDICATIONS BEING SENT TO SCHOOL
 
                  If you send a medication/inhaler/EPI pen to school, it must be in the original container 
                  with name of student, directions,dosage etc. on the medication. I also need the proper 
                  paper work filled out by the physician and parent in order to dispense any medication.
                  ( You can obtain the proper forms on this web page if you look above.)

I am sending home notices to those students who will be entering Kindergarten,  Grade 4 and Grade 7 in September 2017. It is a state law for these grades especially, to have proof of current physicals and immunizations.

All students who have medication in school were sent home with a notice for parents to pick up medication by 6/21/17. We can not hold on to any medication throughout the summer. by thatAny meds that are not picked up  date, will be disposed of properly. If you have any questions, please do not hesitate to call me.

We are hoping for a healthy, accident free school year.
Here are a few reminders to parents/guardians:
  * Please send me any updated physical and immunization records as soon as you get them

  * If your child attends the After School Program and needs any medication, we will need additional doses for that purpose.
